Lifeline to Lifetime

Joseph & Mary’s Home is Northeast Ohio’s first and only medical respite, offering medically-fragile adults experiencing homelessness the help they need to get back to health.

Every day, hospitals throughout Northeast Ohio discharge hundreds of individuals. Most head home to loved ones and continue their recovery in warm, comfortable environments. Others have no home to head to. Too sick for the streets or shelter and not sick enough to require hospital care, these situations can quickly become a matter of life and death.

We serve adults experiencing homelessness who are medically ready to leave the hospital but not well enough to recover on the streets or in a traditional shelter setting. Our goal is to stabilize each resident’s health so they can recover safely, return to self-care, and — if interested — begin a path toward housing stability.

As a ministry of the Sisters of Charity Health System, our sole mission is to serve and care for those in need — to heal their bodies, minds and souls — and be a beacon of hope for our most vulnerable people.

Caring for Each Person

Healing requires nourishment, rest and a sense of security. It requires dignity and advocacy.

At Joseph & Mary’s Home, we provide individual care for each person and get to know their dreams for the future. Support starts with 24/7/365 staffing at a 10-bed facility for women and an 11-bed facility for men. We also offer:

• Holistic care beyond physical sickness to help residents heal emotional and spiritual wounds.
• Collaborations with insurers, hospitals, housing providers, agencies and researchers to find effective approaches that advance the care we provide.
• If interested, connection to housing resources to ensure stability after leaving the program.

Medical Respite

The medical respite model fills a service gap between hospital and shelter. Medical respite programs serve people experiencing homelessness who are ready to be discharged from a hospital but too frail to recover on the streets or in shelters. The goal of respite care is to stabilize homeless patients’ medical conditions so they can transition safely to self care. With this model, Joseph & Mary’s Home residents qualify for additional housing options, including permanent supportive housing—an important solution for the chronically homeless who need housing with additional support systems.
